    New user starting date.

    Phone Number of the new user
    Name of the person that the new joinee will report to.

    Copy permissions from the person being replaced to the new user?
    Please enter the full name of the person that this person is replacing. If they are not replacing anyone, write “none”.
    If you have any additional replacement notes please write them here.
